Self-Determination
At PathPoint, we are proud to support individuals participating in California's Self-Determination Program (SDP) - a transformative initiative that empowers people with intellectual and developmental disabilities to have greater control, flexibility, and choice in the services and supports they receive.
What is the Self-Determination Program?
The Self-Determination Program, offered through California’s Department of Developmental Services, allows eligible individuals to design and direct their own services to achieve their personal goals. Participants work with their Independent Facilitator, Financial Management Services (FMS) agency, and other support team members to create a person-centered plan that reflects their unique needs, preferences, and aspirations.
PathPoint’s Role in the SDP
PathPoint is honored to partner with individuals and their support teams to provide high-quality, person-centered services under the Self-Determination Program. We collaborate closely with:
- Individuals and Families - to understand their goals and tailor services that promote independence, inclusion, and personal growth.
- Independent Facilitators - to ensure services align with the person's person-centered plan.
- Financial Management Services (FMS) Agencies - to coordinate service delivery and ensure compliance with program guidelines.
